Bariatric Surgery

Surgical procedures (sleeve gastrectomy, Roux-en-Y gastric bypass, biliopancreatic diversion, adjustable gastric band) that produce substantial sustained weight loss and remission of type 2 diabetes and other obesity-related conditions.

Also: Metabolic Surgery, Metabolic surgery, Weight Loss Surgery, Weight loss surgery

Mechanisms extend beyond restriction/malabsorption to neurohormonal changes (GLP-1, PYY, ghrelin), bile acid signaling, and microbiome shifts. Long-term micronutrient monitoring is essential: B12, iron, folate, vitamin D, calcium, copper, zinc, thiamine, vitamin A, K, and B6. Common complications include dumping syndrome, hypoglycemia, kidney stones (RYGB), and bone loss. The 2022 ASMBS/IFSO guidelines lowered BMI thresholds, recognizing surgery as primary therapy for severe obesity and metabolic disease.

How each textbook covers it

  • Krause and Mahan's Food and the Nutrition Care Process, 16th ed.Chapter 21

Common procedures: Roux-en-Y gastric bypass (RYGB), sleeve gastrectomy (LSG), adjustable gastric banding, biliopancreatic diversion with duodenal switch. Indications: BMI >=40, or >=35 with comorbidities, or >=30 to 34.9 with poorly controlled T2DM (per recent ASMBS/IFSO update). Lifelong micronutrient supplementation is required to prevent deficiencies (B12, iron, calcium, vitamin D, thiamin, folate, copper, zinc).
